  This is an official form for individuals who have already met with our RCIA coordinator and have decided to go through the RCIA process.   Please provide your information as requested. If you are uncertain, please ask or look it up. We haven't asked for anything we don't need.
  
  When asked for your city/state of birth, please provide the city and state that would be listed on your birth certificate. (For instance, if you were born in a hospital and the hospital was in a different city than your residence, please list the city/state of the hospital.)
  
  When asked for the legal names of your parents, please list your biological parents, or those who would be listed on your birth certificate. (If your parents are unknown, please list the information recorded on your birth certificate, or a similar legal document.)
  
  When asked for your baptismal information, please provide the exact date, name of the church, and full address. If you do not have all of this information, you may need to provide a witness statement from someone who witnessed your introduction into God's family. Please provide the full name of the priest, deacon, or other individual who baptized you.
  
  If you have not been baptized, please provide the anticipated date of baptism (i.e. Easter vigil of this year) and the name and address of St. Vincent de Paul Catholic Church (2375 E Arizona Ave).
